MindLight Antidepressant Response Test: A New Era in Personalized Depression Care

MindLight™, a recently launched, novel blood test, represents a breakthrough in MDD treatment by offering a personalized roadmap to guide treatment decisions and improve patient outcomes. This innovative test identifies a specific circular RNA biomarker (circRNAx) in the blood that has been shown to be highly predictive of response to SSRIs, the most commonly prescribed class of antidepressants. This brain-derived biomarker was identified and validated through extensive research in the large-scale EMBARC clinical trial. Patients categorized as having a “High Likelihood of Response” to SSRIs based on their MindLight test results were over 3.4 times more likely to respond to SSRIs than those classified as “Low Likelihood of Response.”
MindLight differs from other types of tests used to guide depression treatment. Pharmacogenomics (PGx) tests are available that assess genetic variations that can affect drug metabolism and potential interactions. However, any improvement in treatment outcomes from PGx testing remains under dispute. A 2024 task force with the American Psychiatric Association concluded that "evidence does not support the use of currently available combinatorial PGx tools for treatment selection in major depressive disorder." More recently, the large national payer, United Healthcare, announced that they would no longer pay for multigene panel pharmacogenomics tests of five or more genes to guide treatment of psychiatric conditions beginning January 1, 2025. United Healthcare cited the lack of evidence supporting the use of multigene or combinatorial PGx tests in guiding treatment of these conditions. MindLight, on the other hand, directly measures a biomarker that is associated with the biological mechanisms underlying SSRI response.

MindLight streamlines the MDD treatment process in several ways. The Science Behind MindLight

MindLight reflects brain activity related to neurotransmitters involved in response to SSRIs. Circular RNA (CircRNAs), including circRNAx, possess the unique ability to cross the blood-brain barrier. Their presence and abundance in the blood provide a valuable window into brain function and can serve as reliable indicators of neurological processes relevant to MDD and treatment response. The circRNAx biomarker measured by MindLight is directly correlated with levels of serotonin and brain-derived neurotrophic factor (BDNF) (Papageorgiou et al, BioRxiv, 2024). These neurotransmitters are known to play a key role in modulating response to SSRIs. Thus, lower baseline levels of circRNAx indicate a higher likelihood of response to SSRIs, as these medications increase serotonin and BDNF, which in turn elevate circRNAx levels. This biological connection strengthens the test's predictive value.
The results of clinical studies have shown that MindLight can accurately predict SSRI response with a Positive Predictive Value (PPV) of 77%, meaning that 77% of patients identified as having a "High Likelihood of Response" respond to SSRIs (Papageorgiou et al, BioRxiv, 2024). This is significantly higher than the current average response rate of 41% for SSRIs across 36 clinical trials, highlighting the potential of MindLight to significantly enhance treatment outcomes.
